This information packet outlines the procedures and contains the forms needed for an individual and/or congregation to participate in the Deacon Program of the Mid-South District.

The packet/program contains:

  1. Agreement Forms for Deacon Ministry
  2. Annual Ministry Agreement Renewal Forms
  3. Deacon Application Form
  4. Supervising Pastor Annual Report

Directions:

The downloaded file, Deacon.exe, is a database for the Deacon program.

Be sure to follow/print the following directions in order to use this program:

Once an end user downloads the file, double click on the Deacon.exe icon to install the program.

Once it is installed, you can access it by going to Start – Programs – Deacon – Deacon.
The password is: Deacons.

All deacons are asked to complete the application form even if you have been serving as a deacon prior to this time. This will allow the District Office to build a data base for the Deacon program with the most current information possible on it.

To complete your Annual Ministry Agreement Renewal Form and Supervising Pastor Annual Report Form, please go to those forms on your computer that are found under the Layout dropdown menu on the left side of the screen. Print a hard copy of the forms and have them signed by the appropriate people and mail to the District Office.
